Mission
Create a site for healthcare providers and other patient advocates so they can learn and work together towards the development of safer health systems and higher quality of patient care.
History
Concerns about the quality and safety of patient care have now been present for decades. Many people, including medical professionals have been impacted my medical error. Sometimes they are the recipient of an error, other times they may feel that they are the instigators of the error.
Despite decades of work to improve the safety of care, the constant churn of people, new care modalities and a wealth of new technologies make the healthcare environment a complex adaptive system. Change is constant and harm can emerge in unexpected ways. Individual vigilance is not enough to ensure safe care. Instead we have must develop ways to engage as teams with a constant focus on patient outcomes.
